Treadwell’s website is primarily an informational website. We do not provide a
website contact form, discussion board, blog, or online appointment-scheduling system.

We may collect limited technical information about visitors as part of operating
and maintaining the website and providing website analytics. This information may

• Pages visited;
• Date and time of visits;
• Referring website;
• Browser language; and
• General geographic or usage information.

We may also receive information that you voluntarily submit through the
Thrizer insurance-benefits verification tool, as described below.

2. Jetpack Statistics

Treadwell uses Jetpack Stats, provided by Automattic, to obtain
limited information about how visitors use our website.

browser and device information, pages visited, referring URL, date and time of
visits, browser language, and general geographic information.

We use this information to understand website traffic and performance and to
improve the website. We do not use Jetpack Stats to maintain or analyze
counseling records.

3. Thrizer Insurance Benefits Verification

Our website includes an embedded Benefits Widget provided by Thrizer, Inc.
The widget allows prospective clients to check their potential out-of-network
insurance benefits before beginning services.

If you choose to use the widget, you may be asked to provide information including:

• First and last name;
• Date of birth;
• Insurance company;
• Insurance member ID;
• Provider; and
• Service requested.

Thrizer uses this information to verify out-of-network insurance benefits and
provide information regarding potential benefits and reimbursement.

Information you enter into the widget is transmitted to and processed by Thrizer
and may constitute personal information and/or protected health information.

Thrizer’s Privacy Policy states that it may collect health insurance plan data
and other protected health information, including mental and behavioral health
appointment and claim data and diagnostic data.

Thrizer also states that it may disclose information to contractors, service
providers, and other third parties that support its business, subject to applicable
contractual obligations.

Treadwell does not control Thrizer’s collection, use, storage, or disclosure of
information submitted through the Benefits Widget. Information submitted through
the widget is also subject to Thrizer’s privacy practices.

Please provide only the information necessary to obtain an insurance-benefits
estimate.

Insurance-benefit estimates are not guarantees of payment or reimbursement.
Your actual benefits may differ based on your insurance plan, deductible,
coverage, claims processing, and other factors.

4. Client Portal

Treadwell’s website contains a link to a separate client portal operated through
Sessions Health.

The client portal is separate from the public Treadwell website. Information
submitted through the client portal is subject to the privacy and security
practices applicable to that service.

If you are a current client, please use the client portal rather than the public
Treadwell website to communicate information that requires secure handling.

5. Cookies and Similar Technologies

Our website may use cookies or similar technologies necessary for website
functionality and for Jetpack Stats.

Cookies are small files that may be stored on your device when you visit a website.

You may control or disable cookies through your web browser settings. Disabling
certain cookies may affect the functionality of some website features.

Third-party services embedded in or linked to our website, including Thrizer,

6. Information We Do Not Collect Through the Public Website

The public Treadwell website does not provide a mechanism for submitting:

• Therapy session notes;
• Treatment plans;
• Psychological assessments;
• Detailed mental-health histories;
• Therapy communications; or

The website does, however, provide access to third-party services, including
the Thrizer Benefits Widget and the Treadwell client portal, which may collect
sensitive information as described in their respective privacy policies.

7. SMS Text Messaging and Mobile Information

If you provide your mobile telephone number to Treadwell Counseling, PLLC and opt in to receive text messages, we may use your mobile number to send appointment reminders and confirmations, scheduling or rescheduling communications, responses to inquiries, and other administrative or practice-related communications.

Message frequency varies. Message and data rates may apply.

No mobile opt-in information, SMS consent information, or text messaging consent will be sold, rented, or shared with third parties or affiliates for marketing or promotional purposes.

Information may be disclosed to service providers that assist Treadwell Counseling in providing text messaging services, but only as necessary to provide those services and subject to applicable privacy and confidentiality requirements.

Consent to receive SMS text messages is optional and is not a condition of receiving counseling, mental performance coaching, or other services from Treadwell Counseling.
